Good Catch Patient Safety award Nomination
The Good Catch Patient Safety Award is presented to an individual who demonstrates their commitment
to keeping patients safe by “speaking up” to prevent a potential patient safety issue.
A Good Catch is defined as an event or circumstance that has the potential to cause an incident or critical incident but that did not actually occur due to corrective action and/or timely intervention.
Nominations will be accepted throughout the year and will be judged by the Good Catch Subcommittee
on the criteria of:
• Likelihood that a patient safety issue would have resulted if the Good Catch did not happen.
• Potential impact to the patient(s) if the Good Catch did not happen.
To nominate any employee or student within the organization for this award, complete the nomination form below, OR Fax: 518.243.4459 OR Email: goodcatch@ellismedicine.org.
